TO APPLY FOR WEATHERIZATION ASSISTANCE
You must…
- Submit the WAP Application All pages completed and signed AND
- Submit the required documents

- Social Security Cards, Medicare card, or any Federal form with SSN – copies of all persons in your household
- If no Social Security Card, you must complete Certification of No Social Security Form
- Proof of Income
- 4 consecutive weeks of paystub if paid weekly with year-to-date information; if paystubs have no year to date gross income, provide 3 months of paystubs. If paid bi-weekly 2 consecutives paystubs are required. MUST BE CURRENT within 30 days of application
- If Unemployed – 4 Weekly Unemployment stubs or BC-5 Form
- All Rental Income
- If No Income, provide your 1040 or you must complete and notarize a No Income Certification form.
- Current 2022 Social Security Award Letters- monthly statement or Bank Statement (SSD/SSI/SSA), Pensions, Alimony, Annuities, Interest or Dividends or Veteran’s Benefit
- Current Food Stamp/TANF Award Letter with stated amount.
- If member is 18 or older and in school full-time, client must provide a Student schedule otherwise must provide income documentation.
- Utilities Bill 2022– current gas/electric or oil bill. ALL PAGES ARE NEEDED. If oil bill, three most recent bills. If gas heating, client must provide consumption report for January to December of previous year from the utility company (showing dollar amount and usage).
- If Homeowner – provide Tax Bill or current mortgage bank statement or Deed (If Mobile Home needs DMV Title and Lease). Landlord/Tenant must fill out Weatherization Forms.
- If Tenant, Landlord Permission form must be completed and signed
