• Submit the LIHEAP Application (between October 1 and June 30) OR the USF Application (anytime throughout the year) AND
• Submit the Required Documents:
  • New Application
    • Application
    • Copies of Social Security cards for everyone in the household
    • Current Utility bills ALL PAGES
    • Current Lease agreement or property tax/mortgage statement
    • Rental Assistance (if applicable)
    • Current Consecutive Income for everyone 18 yrs old and older
  • Recertification
    • Recertification Form: English Español
    • Current Utility bills ALL PAGES
    • Rental Assistance (if applicable)
    • Current Consecutive Income for everyone 18 yrs old and older
• Send:
  • Via US Mail: 754 Route 18, Suite 202, East Brunswick, NJ 08816
  • In person: Drop off at either at our Perth Amboy OR East Brunswick office
  • At an Outreach Event or Intake Session: Provide documentation directly to our Outreach Representative

• If you are over the low-income threshold for LIHEAP/USF, you may qualify for a Payment Assistance for Gas and Electric (PAGE) grant of up to $750 (depending on the balance owed)
